This Third-Party Telehealth Disclosure ("Disclosure") explains the relationship between Vital Mobile IV Therapy ("Vital"), independent healthcare professionals, third-party telehealth providers, and dispensing pharmacies. This Disclosure forms part of Vital's Website Terms of Service and should be read together with our Privacy Policy, Payment, Cancellation & Refund Policy, and any applicable informed consent documents.
Purpose of This Disclosure
- Vital Mobile IV Therapy provides concierge IV therapy services and facilitates access to certain wellness and prescription-related services.
- Some services—including peptide programs and other prescription-based treatments—require an evaluation by a licensed healthcare professional before treatment can be provided.
- This document explains the role of each party involved in your care.
Vital's Role
Vital is responsible for the non-clinical coordination of services, which may include:
- Scheduling appointments;
- Patient support;
- Program coordination;
- Payment processing;
- Customer service;
- Educational resources;
- Communication regarding appointments and services; and
- Coordination with independent healthcare providers and dispensing pharmacies when necessary to facilitate requested services.
Except where Vital directly provides IV therapy services through appropriately licensed clinicians, Vital does not independently diagnose medical conditions, prescribe medications, or make medical treatment decisions.
Independent Healthcare Professionals
Certain services require review by an appropriately licensed healthcare professional. These healthcare professionals may provide services through an independent telehealth organization, medical group, or other healthcare entity. The treating healthcare professional is solely responsible for all clinical decisions, including:
- Reviewing your medical history;
- Determining whether additional information or laboratory testing is required;
- Determining whether telehealth is appropriate;
- Determining whether treatment is medically appropriate;
- Prescribing medications;
- Selecting medications and dosages;
- Monitoring therapy;
- Modifying treatment; and
- Discontinuing treatment when clinically appropriate.
Vital does not control or direct the independent professional judgment of the treating healthcare professional.
Third-Party Telehealth Providers
Vital may facilitate access to an independent third-party telehealth platform, such as Qualiphy, to assist with patient intake, medical evaluations, and coordination of healthcare services. Independent telehealth providers may:
- Collect medical information;
- Conduct telehealth evaluations;
- Review medical records;
- Request additional information;
- Coordinate with licensed healthcare professionals; and
- Communicate treatment recommendations or eligibility decisions.
Use of an independent telehealth platform may also be subject to that organization's own Terms of Service, Privacy Policy, Notice of Privacy Practices, Telehealth Consent, and other required documentation. Patients may be required to review and agree to those documents separately before receiving services.
Telehealth Services
Telehealth involves the delivery of healthcare services through secure electronic communication when the patient and healthcare professional are not in the same physical location. Telehealth may include:
- Secure video visits;
- Telephone consultations where permitted;
- Electronic questionnaires;
- Secure messaging;
- Review of submitted photographs or documents; and
- Electronic transmission of health information.
Telehealth offers convenience and expanded access to care but may not be appropriate for every patient or every medical condition.
Potential Limitations of Telehealth
Patients understand that telehealth has limitations, including:
- The inability to perform a complete physical examination;
- Technical interruptions or connectivity issues;
- Delays in communication;
- The possibility that additional testing or an in-person evaluation may be required; and
- Circumstances where telehealth is not an appropriate method of care.
A healthcare professional may recommend an in-person evaluation before approving treatment.
No Guarantee of Treatment
Submitting information, scheduling a consultation, participating in a telehealth evaluation, or paying for a program does not guarantee that:
- You are medically eligible for treatment;
- A prescription will be issued;
- A specific medication will be prescribed;
- A medication will be available through a dispensing pharmacy; or
- Any particular clinical outcome will occur.
Dispensing Pharmacy
If treatment is approved, prescription medications may be filled by an independent licensed dispensing pharmacy. The dispensing pharmacy is responsible for:
- Preparing or compounding medications where applicable;
- Dispensing medications pursuant to a valid prescription;
- Labeling;
- Packaging;
- Shipping; and
- Providing medication-specific instructions.
Vital does not manufacture prescription medications.
Patient Responsibilities
Patients agree to:
- Provide complete and accurate medical information;
- Disclose medications, allergies, and relevant medical history;
- Inform the healthcare professional of significant changes in health status;
- Follow treatment instructions;
- Complete recommended laboratory monitoring when required;
- Report concerning side effects promptly; and
- Ask questions whenever additional clarification is needed.
Providing incomplete or inaccurate information may affect treatment decisions and patient safety.
Emergencies
Telehealth services facilitated through Vital are not intended for medical emergencies. If you believe you are experiencing a medical emergency, call 911 or seek immediate emergency medical care.
Privacy
Information shared during the telehealth process may be disclosed among Vital, the independent healthcare professional, the telehealth platform, and the dispensing pharmacy only as reasonably necessary to coordinate requested services and as permitted by applicable law. Additional privacy practices may be governed by the independent telehealth provider's own Privacy Policy and Notice of Privacy Practices.
