1. Agreement
These Peptide Program Terms & Conditions ("Terms") govern your participation in the peptide therapy programs offered through Vital Mobile IV Therapy ("Vital," "we," "our," or "us"). These Terms supplement Vital's Website Terms of Service, Payment, Cancellation & Refund Policy, Third-Party Telehealth Disclosure, Privacy Policy, and any applicable informed consent documents. If these Terms conflict with another document governing your peptide program, the more specific document controls with respect to the peptide program.
2. Eligibility
Participation in Vital's Peptide Program requires all of the following:
- You must be at least eighteen (18) years of age.
- You must be legally eligible to receive healthcare services in your state.
- You must complete all required intake forms and medical questionnaires.
- You must be evaluated by an appropriately licensed independent healthcare professional.
- You must satisfy all medical requirements established by the treating healthcare professional.
- At this time, peptide programs are available only in locations where Vital and its independent healthcare providers are authorized to offer such services.
3. Independent Medical Decision-Making
Vital coordinates peptide programs but does not practice medicine or direct medical decision-making. All clinical decisions are made solely by an independent licensed healthcare professional. This includes decisions regarding:
- medical eligibility;
- diagnosis;
- treatment recommendations;
- laboratory testing;
- prescription approval;
- medication selection;
- dosing;
- treatment duration;
- monitoring requirements;
- refill authorization; and
- discontinuation of therapy.
Payment for a peptide program does not guarantee approval for treatment or issuance of a prescription.
4. Telehealth Evaluation
Participation in the Peptide Program requires completion of a medical evaluation by an independent healthcare professional, which may occur through an approved third-party telehealth platform. Patients may be required to review and accept separate telehealth consent forms, informed consent documents, Notices of Privacy Practices, and other documentation required by the healthcare provider or telehealth platform.
5. Medical Eligibility
The treating healthcare professional may determine that:
- treatment is appropriate;
- additional information is required;
- laboratory testing is required;
- additional follow-up is necessary; or
- treatment is not medically appropriate.
Vital has no authority to alter or override these clinical decisions.
6. Prescription Medications
If treatment is approved:
- prescription medications will be prescribed only by an appropriately licensed healthcare professional;
- medications may be dispensed by an independent licensed pharmacy;
- certain medications may be compounded when permitted by applicable law; and
- medication availability may change without notice due to pharmacy, regulatory, or supply considerations.
Vital does not manufacture prescription medications.
7. Patient Responsibilities
Patients agree to:
- provide complete and accurate medical information;
- disclose all current medications, supplements, and allergies;
- disclose pregnancy, breastfeeding, or plans for pregnancy where applicable;
- disclose relevant medical conditions;
- disclose significant changes in health status during treatment;
- complete requested laboratory testing;
- attend required follow-up appointments;
- follow dosing instructions;
- store medications according to pharmacy instructions;
- promptly report significant side effects; and
- ask questions whenever clarification is needed.
Providing inaccurate or incomplete information may result in delayed treatment, denial of treatment, or discontinuation of therapy.
8. Informed Consent
Participation in the Peptide Program requires completion of one or more separate informed consent documents. These Terms do not replace medical informed consent. Patients are responsible for reviewing all informed consent materials and discussing questions with the treating healthcare professional before beginning therapy.
9. Refills
Prescription refills are not automatic. Continuation of therapy is determined solely by the treating healthcare professional. Additional requirements before a refill may include:
- follow-up medical evaluation;
- updated medical history;
- laboratory testing;
- review of treatment response;
- review of side effects; or
- additional documentation.
10. Laboratory Monitoring
Certain peptide therapies require periodic laboratory testing or other clinical monitoring. Patients are responsible for completing recommended monitoring within the timeframes established by the treating healthcare professional. Failure to complete requested monitoring may result in delayed or discontinued treatment.
11. Shipping
Prescription medications are shipped directly by the dispensing pharmacy. Patients are responsible for:
- providing an accurate shipping address;
- monitoring shipment tracking information;
- receiving shipments promptly;
- following refrigeration requirements where applicable;
- storing medications according to pharmacy instructions; and
- inspecting shipments promptly upon delivery.
Vital is not responsible for delays caused by weather, shipping carriers, pharmacies, manufacturers, supply shortages, or other circumstances outside Vital's reasonable control.
12. Returns and Refunds
Because peptide medications are prescribed for an individual patient and may include compounded medications:
- prescription medications may not be returned;
- prescription medications may not be exchanged;
- medication charges become non-refundable once the prescription has been transmitted to the dispensing pharmacy for processing or fulfillment, except where required by law.
Payment and refund terms are further governed by Vital's Payment, Cancellation & Refund Policy.
13. Program Changes
Vital reserves the right to modify or discontinue any peptide program, including:
- available medications;
- dispensing pharmacies;
- pricing;
- program structure;
- membership offerings;
- required documentation; and
- administrative procedures.
Nothing in this section permits Vital to interfere with the independent clinical judgment of the treating healthcare professional.
14. Suspension or Termination
Vital or the treating healthcare professional may suspend or discontinue participation in the Peptide Program if:
- treatment is determined to be medically inappropriate;
- required laboratory monitoring is not completed;
- inaccurate or misleading medical information is provided;
- program requirements are not followed;
- fraudulent activity is suspected;
- abusive or threatening behavior occurs toward staff or healthcare professionals; or
- continued participation presents safety, legal, or operational concerns.
Termination of participation does not automatically entitle a patient to a refund.
15. No Guarantee of Results
Individual responses to peptide therapy vary. Vital makes no guarantee regarding:
- weight loss;
- body composition;
- muscle retention;
- muscle gain;
- athletic performance;
- recovery;
- energy;
- cognitive performance;
- sleep;
- hormone levels;
- healthy aging;
- longevity;
- inflammation;
- appearance; or
- any other health outcome.
Testimonials and patient experiences presented by Vital represent individual experiences only and should not be interpreted as guarantees of future results.
16. Patient Communications
Vital may communicate with patients regarding:
- appointment scheduling;
- refill coordination;
- payment matters;
- educational materials;
- laboratory reminders;
- pharmacy coordination; and
- other administrative matters related to participation in the program.
Marketing communications remain subject to the patient's communication preferences and Vital's SMS Terms & Conditions and Privacy Policy.
17. Limitation of Program Availability
Participation in the Peptide Program is subject to:
- applicable federal and state law;
- pharmacy availability;
- provider availability;
- medication availability;
- clinical appropriateness; and
- operational capacity.
Vital does not guarantee continued availability of any specific medication, dosage, formulation, or pharmacy.

By enrolling in or requesting participation in the Vital Mobile IV Therapy Peptide Program, you acknowledge that you have read and understand these Terms & Conditions. You further acknowledge that participation in the program remains subject to the independent clinical judgment of the treating healthcare professional and any additional informed consent or telehealth documentation required before treatment begins.
